Strange noise from the engine suddenly

That is the balancer chain adjustment - the cam chain tensioners are automatic unless one is defective. There are two cam chain tensioners, one for each cam. It could be that you have a bad cam chain tensioner - it may be worth investigating. The tensioners can be manually manipulated with a small screwdriver. Also the balancer shaft chain adjustment can be fairly finicky to get right. If you have tried adjusting it already it may be worth another go at it.
 
For the cam chain tensioners, how can i test them?
Middle screw off, and wind them up clockwise and then release?

The tensioners are self-adjusting. Before you go playing with them, the first thing i would do is pull the valve covers and see if one of the chains have excessive slack. If so, you know where to start looking. Manipulating the tensioners, without understanding how they work, COULD end up in disaster.
